Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/05/2025. The on-site inspection began at 9:25 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 34 children present and 8 staff providing direct care. The inspector reviewed 5 children?s records and 5 staff records on-site on 03/05/2025.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain a completed sworn statement prior to the employee?s first day of employment.

The record for Staff 5, who has been employed for more than 3 months, did not contain a completed sworn statement.
Plan of Correction: Will get it done today.
Standard 8VAC20-780-160-A
REPEAT VIOLATION

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children and the screening shall have been completed within the last 30 days of employment.

The TB screening for Staff 4 was conducted 8 months prior to employment.
Plan of Correction: Moving forward, will make sure that the TB screening is not more than 30 days old.

Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

The record for Staff 4, who has been employed for over 1 month, did not contain documentation of completing orientation training. The record for Staff 5, who has been employed for over 3 months, did not contain documentation of completing orientation training.
Plan of Correction: Will review with the staff and place into the record.

The center shall maintain a separate record for each enrolled child that contains all of the information required by the standards.
