An unannounced monitoring inspection was initiated on 04/10/2025 and completed on 04/28/2025 in response to a self-report received by the licensing office on 04/10/2025 relating to staffing and supervision and special care provisions and emergencies. The inspector reviewed 1 child?s record and 1 staff record on-site on 04/14/2025.
The inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center shall have a physical examination by or under the direction of a physician by the child?s attendance or within 30 days after the first day of attendance.
The record for Child 1 did not contain a physical examination; Child 1 has been attending for more than 2 years.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 04/09/2025, Staff 1 spun Child 1 (2 years old) around in a swivel desk chair. Child 1 made approximately 27 rotations in the chair. When Child 1 stood up, they fell face forward, striking their chin on the floor. The fall resulted in a wound to the chin that required outside medical treatment.
