Inspector Notes

An unannounced inspection was initiated on 08/04/2025 and completed on 08/12/2025 in response to a complaint received by the licensing office on 07/25/2025 relating to allegations of the subsidy program vendor requirements. The inspector reviewed 2 children?s records on-site. The inspection included document review, tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable subsidy requirement and the violations are documented on the violation notice issued to the program.

The vendor shall ensure compliance with the terms of the vendor agreement; the vendor shall not be in possession of or swipe any parents? or guardians? VA-ECC card.

Staff 5 confirmed that they had been in possession of and swiped the VA-ECC card for Child 1 and Child 2.
Plan of Correction: Will not assist parents with their cards.
