Inspector Notes
An unannounced monitoring/renewal inspection was conducted today between 10:00am and 11:20am. There were 9 children present with the Provider and 2 assistant for a total of 18 points within the supervision guidelines. The children were observed laying out their work stations, doing puzzles, working in educational workbooks, looking at growing baby chicks in their eggs and other organized activities. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. All staff are currently trained to administer medication. One medication and authorization was reviewed. Violations were found during today?s inspection and can be viewed on the Violation Notice.

3Evidence: Household member #3's sworn disclosure statement confirmed that they had resided outside of Virginia in the last five years. The provider confirmed that they didn't realize the household member had lived out of the state and hadn't completed the additional out-of-state background checks.
Evidence: The provider and household member #1's sworn disclosure statements expired on 11/17/2022. The provider confirmed that they had not yet completed updated sworn disclosure statements.
Evidence: The findings of the most recent inspection were not posted. The provider stated that the inspection report was in storage in the basement.
