Inspector Notes

An unannounced renewal inspection was conducted on October 3, 2022 from approximately 12:40 pm to 1:30 pm. There were 4 children in attendance. 1 staff record, 2 household member records and 2 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected.

Interviews were held with the provider throughout the inspection. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys to meet the needs of the children. Children were observed taking a nap while the licensing inspector was at the home.

The license and current inspection were posted. Evacuation drills were conducted monthly. Shelter in place drill conducted two times with the most recent on 09/11/22.

The menu was posted.

The center?s first aid kit was inspected and found complete. The home had a working flashlight and battery operated radio.

Based on a review of household member records and interview, the provider did not ensure to have a Sworn Statement of affirmation and central registry results available for each household member.
Evidence: The record of household member #1 did not contain documentation of a sworn statement of affirmation and central registry results. The provider reported the sworn statement was not completed and central registry had been mailed but the results had not been received.

Based on a review of records, and interview, the provider failed to ensure that each adult household member has a current Report of Tuberculosis screening or a form consistent with it documenting the absence of tuberculosis in a communicable form. Evidence: The record for household member #1 did not contain evidence of tuberculosis screening. Household member #1 was seen in the household. The provider reported that the tuberculosis screening had not been completed.
