Inspector Notes
An unannounced monitoring inspection was conducted on March 4, 2024 from approximately 11:00 am to 12:45 pm. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record, 2 caregiver records, 1 household member record, and 6 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were 6 children in attendance for a total of 16 points. The children were observed eating snack, playing in the playroom, and taking a nap, while the inspector was on the premises.
The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted.
The information gathered during the inspection determined violations with applicable standards or law.
Violations were issued.

6Evidence: When the Licensing Inspector arrived caregiver #3 was alone with 6 children. The record for caregiver #3 did not contain the results of a criminal background fingerprints check.
Evidence: The record for child #6, date of enrollment 12/11/23, did not contain documentation of a physical examination.
Evidence:
1. The record for caregiver #2 did not contain a tuberculosis screening.
2. The record for caregiver #3 did not contain a tuberculosis screening.
Evidence: When the Licensing Inspector arrived to the home caregiver #3 was alone with 6 children. The record for caregiver #3 did not contain documentation of current certification in CPR.

Based on interview, the provider failed to obtain a landline telephone that is operable and accessible during the family day home's hours of operation.
Evidence: The provider stated that she does not have a landline telephone.
