Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on July 21, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were three children(9 pts) present, ranging in ages from 11 months to 2 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of three child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #3's record did not contain a street address for a second emergency contact.
Plan of Correction: The provider will obtain the address from the parents today.

Based on observation and interview, telephone numbers were not posted in a visible area close to the telephone.
Evidence:
1. Emergency numbers such as 911, police, fire, and emergency medical responders telephone numbers were not posted.
2. The responsible person for emergency backup care phone number was not posted.
3. The regional poison control center phone number was not posted.
