Inspector Notes

An unannounced monitoring inspection was conducted on March 14, 2024 between the hours of 10:30 am through 11:30am. There were a total of seven children (24 points) observed with three caregivers. A total of five children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, snack, coloring, and other organized activities. Standard 8VAC20-800-180-A
Based on record review, the provider did not obtain for each caregiver a current Report of Tuberculosis(TB) Screening form every two years.
Evidence:
1. Caregiver #3's record did not contain a current TB Screening form. The record contained documentation of an expired Tb screening dated 06/17/2021.
Plan of Correction: The caregiver will set an appointment to get a new TB screening.
Standard 8VAC20-800-750-A
Based on observation, diaper ointment was not labeled with child's name.
Evidence:
1. Child #2's and Child #4's diaper cream was not labeled with their names.
Plan of Correction: The diaper creams will be labeled with each child's name.
Standard 8VAC20-800-830-A
Based on documentation review and interview, the emergency evacuation procedures were not practiced monthly with all caregivers and children in care during all shifts that the children are in care.
Evidence:
1. The emergency evacuation drills were not conducted for the months of January and February.
2. Caregiver #1 confirmed that the January and February drills were not conducted.
Plan of Correction: The provider will conduct two additional emergency evacuation drills during the month of March to make up for the two missed drills.
Standard 8VAC20-800-940-A
Based on observation, temperatures were not maintained at or below 40 degrees Fahrenheit in refrigerator compartments.
Evidence:
1. The temperature of the refrigerator had a thermometer reading of 46 degrees Fahrenheit at the time of the inspection.
Plan of Correction: The refrigerator's dial was switched to the coldest setting to ensure that the temperature was maintained at 40 degrees Fahrenheit or below at the time of the inspection.
