Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on September 20, 2024, as a part of licensure period. The on-site inspection began at 9:30am and ended at 11:00am. The inspector reviewed compliance in the areas listed above.There were nine children present and four caregivers; the point total was 32. The inspector reviewed six children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-750-A
Topical products such as diaper cream require annual written parent authorization noting any adverse reactions.

Written parent authorization for diaper cream was not available for Child #2.
Plan of Correction: The provider will have the parent complete written authorization for the child's diaper cream.
Standard 8VAC20-800-80-B
The provider shall verify the identity and age of a child by viewing the certified birth certificate within seven business days of the child's first day of attendance.

The birth certificate for Child #5, who has been in care for 13 business days, was not viewed by the provider .
Plan of Correction: The provider will verify the identity and age of the child by asking the parents to view the original birth certificate.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.

Two bottles for Child #4 were not labeled with the child's full name and date. There were five infants in care.
Plan of Correction: The provider labeled the bottles at the time of the inspection.
