Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 4, 2025, as a part of the licensure period.The on-site inspection began at 11:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and three caregivers; the point total was 37. The inspector reviewed five children?s records and four caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Children's records are required to have name, address, and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached and a policy number of the child's medical insurance.

The record for Child #1 was missing the policy number of the child's medical insurance. Child #1 was present during the inspection and has been in care for five months. The records for Child #2 and Child #3 did not have the name, address, and telephone number of a second emergency contact. Child #2 and Child #3 were present at the time of the inspection. Child #2 has been in care for six days. Child #3 has been in care for two months.
Plan of Correction: The provider will get the policy number for Child #1 and the second emergency contact for Child #2 and Child #3. The provider will ensure that the parent has completed all the required information on the child record.
