Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 2, 2025, as a part of the licensure period. The on-site inspection began at 9:50am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were 11 children present and three caregivers; the point total was 34. The inspector reviewed six children?s records and four caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Repeat Violation

Children's records are required to contain name and policy number of the child's medical insurance and name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

The record for Child #3 was missing the policy number of the of the child's medical insurance. Child #3 has been in care for one month. The record for Child #4 was missing the name and policy number of the child's medical insurance. Child #4 has been in care for five months. The record for Child #5 was missing the name, address, and telephone number of a second emergency contact. Child #5 has been in care for less than a month.
Plan of Correction: The provider will obtain the name and policy number of the children's medical insurance from the parents today. Also, the name, address, and telephone number of a second emergency contact will be obtained from the parent today.
Standard 8VAC20-800-80-A
Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the identity and age of the child.

Documentation of verification of the identity and age of Child #1, Child #2, Child #3, and Child #4 was not obtained from the provider within seven business days of the first day of attendance.
Plan of Correction: Verification of the identity and age of the children will be obtained from the parents by the next day of business. Moving forward, the provider will ensure to obtain verification of the identity and age of the child within seven business days of the first day of attendance.
