Inspector Notes
An investigation was initiated from 2:00 pm - 3:00 pm to investigate a complaint regarding a forbidden action. Interviews were conducted, documentation, and video footage was reviewed. The complaint regarding forbidden actions is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interviews and video footage it was determined staff engaged in rough handling which is a forbidden action. Evidence: On 5/11/2023 at around 11:30 am three children can be seen on the carpet playing near a bookshelf. Child A (Age: 21 months) appears to move towards child B (Age: 20 months) in an attempt to bite child B. Staff A can be seen on video pushing child C (Age 23 months) to the side, causing them to fall to the ground, in an attempt to intercept the bite between child A and Child B. Staff A then pulls forcefully on child A causing child A and child B to fall to the floor. Staff A can then be seen swatting child A's arm away.
Plan of Correction: Staff A was terminated and all staff were retrained regarding appropriate behavioral guidance methods.
