Inspector Notes

A monitoring inspection was conducted today with the provider. There were no children present during this inspection. Two children and the provider's files were reviewed. There were no medications nor have their been any injuries/accidents requiring documentation. There was one violation outlined on the violation notice.

Based on a review of two children's files and an interview with the provider, it was determined that there was no physical on file for child A within 30 days of enrollment. Child A enrolled on 2/15/21.
Plan of Correction: I will get the physical before the child returns to my care.
