Inspector Notes
An unannounced monitoring inspection was conducted today with the provider from 9:55 am to 10:40 am. The provider was present with 4 children, ages ranging from 2 to 3 years. The children were actively engaged in story time and were quite familiar with the books being read. The children and provider transitioned to signing and dancing to include using instruments that the children happily played.
Five children and one household member's files were reviewed.
There were no medicaitons on site.
First Aid/CPR certification is through American Heart Association and is due to expire in 7/23.
Please notify your Licensing Inspector of the date you submitted to obtain your background check, which should be within 10 days. Please send a copy of the completed background check upon receipt.

Person A's fingerprint background check is dated 2/16/18 and has not been renewed within 5 years.
Evidence:
Person A's tuberculosis test was dated 8/30/20.
Evidence:
The Licensing Inspector (LI) observed disinfectant wipes, air freshener spray and hand sanitizer on the shelf accessible to children.
Evidence:
Child A, B and C did not have insurance policy numbers on file. Child A and B enrolled on 6/21/22, child C enrolled on 6/21/20.
