Inspector Notes

A renewal inspection was conducted on 8/21/2023 from 1PM to 2PM. 8 children (16pts) were in attendance with 2 caregivers. The physical space, 3 children's records, caregiver and household member documents, the menu, evacuation drills, attendance records and other required posted documents were reviewed. No medications were on site. Children were observed resting. Age appropriate materials were available for the children and process oriented projects were displayed throughout the classroom. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the Violation Notice issued to the program. Based on record review, subsequent criminal background checks were not completed. Evidence: Fingerprinting for Caregiver #1 and Household Member #1 were expired.
Plan of Correction: The Provider will make appointments for fingerprinting right away.
Standard 8VAC20-770-60-B
Based on record review, subsequent Sworn Disclosure statements were not available for review. Evidence: The Sworn Disclosure Statements for Caregiver #1 and Household Member #1 were expired.
Plan of Correction: The Provider will make sure the Sworn Disclosure Statements are updated today.
Standard 8VAC20-800-180-A
Based on observation, a subsequent tuberculosis screening was not obtained in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening. Evidence: For Household Member #1, the latest screening available for review was conducted 9/6/2019, expiring 9/6/2021. The document for a more current screening could not be found.
Plan of Correction: The Provider stated a follow up tuberculosis screening will be completed right away.
Standard 8VAC20-800-60-B
Based on review of records, Child A's record did not include written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. Evidence: The parent did not sign the written authorization for emergency medical care in the file for Child A.
Plan of Correction: Provider will have the parent sign today.
