During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 8/27/25 and completed on 8/27/25, as a part of the licensure period. The on-site inspection began at 9:02am and ended at 10:20am. The inspector reviewed compliance in the areas listed above. There were eight children present and two caregivers; the point total was twenty-one. The inspector reviewed five children?s records and three caregiver/household members records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
REPEAT VIOLATION:

The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

The physical exam record was missing for Child #2 who has been enrolled for approximately four months.
Plan of Correction: Will obtain the physical for the child from the parent.

REPEAT VIOLATION:
Children's records are required to contain addresses for parent's employment address and phone number and the policy number for medical insurance.

The record for Child #3, who had been in care for 3 years and 5 months, was missing one parent's employment address, employment phone number and the policy number of the child's medical insurance. The child was present during the inspection.
Plan of Correction: Will update the child's records with the missing information.
