Inspector Notes

A virtual inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

This inspection was initiated by licensing staff and concluded on 02/25/2022, between the hours of 10 am to 12:30 pm. There were 43 children present, ranging from infant to preschool-age, with 11 staff supervising, in 5 classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; nutrition; and a tour of the center. A total of 5 children records, and 11 staff records were reviewed.

Information gathered during the inspection determined that there were two (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the program.

Standard 8VAC20-780-160-A-2
Based on review, 1 of 11 staff files indicated that tuberculosis (TB) documentation had not been completed within the last 30 calendar days of the date of employment.

Evidence -
1. On the date of inspection (02/25/2022), Staff #11 had TB documentation on file that was dated - 09/30/2021.
2. The start date for Staff #11 was listed as - 11/29/2021.
Plan of Correction: Updated TB documentation will be obtained for Staff #11.

Based on review, 2 of 11 staff files indicated that it has been more than two years from the date of the most recent TB screening or testing on file at the center.

Evidence -
1. On the date of inspection (02/25/2022), the TB documentation on file for Staff #6 was dated - 08/22/2019.
2. The TB documentation on file for Staff #7 was dated - 09/18/2019.
Plan of Correction: Updated TB documentation will be obtained for both Staff #6, and Staff #7.
