Inspector Notes
An unannounced monitoring inspection was conducted on 10/4/2023 from 10:00 am- 11:35 am. There were 36 children (infants-5yrs.) supervised by 12 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, and playing outside. Areas of non-compliance are identified in the Violation Notice. Violations
5Evidence: There were several areas of ground covering underneath the resilient surfacing that is exposed and lifted, causing a tripping hazard on the 3's and 4's playground and the 2's playground.
Evidence: There was no resilient surfacing at the bottom of the 2's playground slide.
Evidence:
1. In the infant room, the allergy list was not kept confidential.
2. The allergy list was posted on a bulletin door next to the classroom door.
Evidence: Written parent authorization noting any known adverse reactions was not obtained for child #6.
