Inspector Notes

An unannounced monitoring inspection was conducted on 1/23/2024 from 10:00 am-11:45 am. There were 42 children supervised by 11 direct care staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, singing, learning spanish, circle time, and eating snack. Diapering and hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard 8VAC20-780-270-A
REPEAT VIOLATION

Based on observation, areas and equipment of the center, inside were not maintained in a safe and operable condition.

Evidence:
1. In the Geese classroom, there is a white cabinet next to the bulletin board with a broken left door. The left side of the cabinet door is not attached to the hinges, which causes it to completey fall off the cabinet.

2. The diaper changing pad in the children's bathroom across from the director's office is torn in two places.

Based on observation and interview, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Penguins classroom, a container of disinfectant wipes, a spray bottle of disinfectant cleaner and hand sanitizer were stored in a unlocked cabinent.
2. In the Ducklings classroom, a container of disinfectant wipes, a spray bottle of disinfectant cleaner and hand sanitizer were stored in a unlocked cabinent.
3. In the Geese classroom, a container of disinfectant wipes, a spray bottle of disinfectant cleaner and hand sanitizer were stored in a unlocked cabinent.

Based on observation of the Geese classroom, hand washing procedures were not followed.

Evidence:
1. Staff #5 was observed not washing their hands after changing Child #1, Child #3, and Child #5's diaper.
2. Staff #5 returned Child #1, Child #3, and Child #5 to the group without washing their hands after diapering.
3. Child #1, Child #2, and Child #4 were observed not washing their hands after snack.

Based on observation of the Geese classroom, diapering procedures were not followed.

Evidence: Staff #5 in the Geese classroom did not clean and sanitize the diaper changing table after diaper changes for Children #1, #3, and 5.

Based on medication review and interview, medication was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
