During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/28/2025, as a part of the licensure period. The on-site inspection began at 10:30am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 46 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. The updated screening for Staff #2 was completed two weeks past due.
Plan of Correction: TB test results have been obtained. TB test results will be obtained every two years.
Standard 8VAC20-780-260-B
Centers ae required to obtain annual approval from the health department for meeting requirements for water supply; sewage disposal system; and food service, if applicable. An annual health inspection from the health department is overdue by more than 7 months.
Plan of Correction: The center will contact the health department to request an inspection.
Standard 8VAC20-780-320-B
Each restroom area provided for children shall be equipped with soap, toilet paper, and disposable towels. One restroom in the center was not equipped with disposable towels.

Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. The fall zones for three climbing apparatus that contain slides were less than the required six feet on each side and four feet plus the height of the slide in front of the chute. Children were present on the playground and using the equipment.
Plan of Correction: Playground equipment will be moved to account for required fall zones.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The list in the preschool room was posted on a bulletin board in plain sight.
