Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 9/09/2024. The on-site inspection began at 9:55am and ended at 10:55am. The inspector reviewed compliance in the areas listed above. There were (11) children present and (3) caregivers; the point total was (35). The inspector reviewed (2) children?s records and (4) caregiver/household records on-site on 9/09/2024. This inspection included document review, tour of the home and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3Based on review of household records, the provider did not obtain for each adult household member a current report of Tuberculosis screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Household member #3 had a TB test or screening dated 8/26/2022 and had been more than 2 years.
Evidence:
1. The stairs on the left side of the room did not have a gate to prevent children under two years of age to have access to stairs.

1. The provider did not practice the emergency evacuation procedures the month of August 2024.
