During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/03/2026. The on-site inspection began at 9:54am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 caregivers; the point total was 31. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site on 3/03/2026.
This inspection included document review, tour of the home, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-120-B
Providers must maintain a record for each caregiver that contains all required information.

Caregiver #2 who has been caring for children for approximately one month did not the Name, address and telephone number of a person to be notified in an emergency.
Plan of Correction: I will have her fill out the form today.
Standard 8VAC20-800-170-A
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children.

The tuberculosis screening or test for Caregiver 2, who has been caring for children for approximately one month was dated 5 days after the hire date..
Plan of Correction: Going forward, I will make sure I have prior to hire.

Documentation of the orientation shall be signed and dated by the provider and assistant by the end of their first week of assuming job responsibilities.

Caregiver #2 who had been working approximately one month did not have documentation of the orientation. The provider stated the training had been completed.
Plan of Correction: I will have her fill out the orientation form with me.
