Inspector Notes

An unannounced code compliance inspection was conducted on 1/9/2024 from 9:30am to 10:40am. At the time of entrance, 39 children were in care with 7 staff members present. Children were observed doing the following: eating snack, washing hands, reading books, playing on the playground, participating in a group activity counting with the teacher and hand writing work sheets. Interactions between the children and caregivers were positive. The site was clean and organized.. A selection of staff and children records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Based on review of three staff records, the Religiously Exempt Child Day Center did not have the fingerprint results prior to the 1st day of employment for one staff.
Evidence:
1. Staff #2 (start date 5/25/2022) had fingerprint results dated 6/03/2022.
Plan of Correction: I will make sure each staff member has it completed before their start date.

Based on review of three staff records, the Religiously Exempt Child Day Center did not obtain a copy of the results of a criminal history record information check or sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #2(start date 5/25/2022) and Staff #3 (start date 12/06/2021) lived in other states in the last 5 years and did not have the sex offender registry check for any of the States and also did not have the criminal history check for Nebraska or Texas for Staff #3.
Plan of Correction: I will send for them today or tomorrow. I will make sure each staff member has it completed before their start date.
