During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site code compliance inspection was initiated and completed on 2/26/2025. The on-site inspection began at 8:46am and ended at 10:21am. The inspector reviewed compliance in the areas listed above. There were 49 children present with 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on 2/26/2025. This inspection included document review, tour of the facility and observations.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/05/2025. A POC submitted after this date will not appear on the public website.

4The written statement was not provided to the parents of Child #1 who has been enrolled approximately 1 month and 2 weeks.
A staff member stated they have parents sign a health questionnaire for the child prior to arrival and check the children at the door, but they have not been formally trained.
The center's playground equipment had four separate areas with fall zones and did not maintain cushioning material under the playground equipment.
The Three Year Old Classroom had Christmas lights hanging down around the windows in reach of children.
The Four Year Old Classroom had 2 extensions cords plugged in to the other and were creating a tripping hazard and a another cord for a heater was also creating a tripping hazard.
The most recent central registry check was overdue by 1 year and 5 months and the Sworn Disclosure Statement was overdue by approximately 10 months for Staff #5.
