The Religiously Exempt Child Day Center is required to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
In the inside play area, 4 vents on the floor were not attached to the floor creating a tripping hazard.

Plan of Correction: The school has ordered new air vents and will install them upon arrival. We will also keep up to date with any additional maintenance and repairs with safety as the main priority.

Plan of Correction: Going forward the school will be sure to obtain the national criminal background checks before an employee's start date. We will also keep up to date with any changes in requirements and deadlines for staff paperwork.
