Inspector Notes
An unannounced monitoring inspection was conducted on 1/09/2023. The inspector was on site at the family day home from approximately 11:06am to 1:10pm. There was 11 children in attendance and 2 caregivers present. Interviews were held with the caregivers throughout the inspection, and the inspector interacted with the children when appropriate. The yard and all areas of the home were inspected today. Three8 caregiver/household member records and five children?s records were reviewed during this inspection. There were violations found during today?s inspection. See the violation notice for more details.

5Evidence:
The record of caregiver #2 (employment date: 10/5/2022) did not have the following required documentation: address, age, emergency contact information, references and orientation.
Evidence:
The family day home?s fireplace was last inspected 10/13/2021.
Evidence:
1. The family day home did not have an operable landline telephone.
2. The provider stated that they did not have an operable landline telephone in the home.
Evidence:
1. An infant child was observed sitting in a Bumbo Seat on the home?s kitchen table for approximately 10 minutes. The child was not strapped into the seat.
2. The manufacturer?s instructions state the following: ?Always use with the restraint belt.?, ?Only use the seat on floor or level ground surface.? and ?Never use on raised surfaces such as tables, counters, chairs, beds or near stairs.?
Evidence:
There was a KAKIBLIN Newborn lounger located in a crib. There was a sleeping infant using the padded/stuffed lounger during the inspection. Once notified of the hazard, the provider immediately removed it.
