An unannounced, on-site monitoring inspection was completed on 11/25/2024. The on-site inspection began at 11:33 a.m. and ended at 2:50 p.m. The inspector reviewed compliance in the areas listed above. There were 8 children present, 2 caregivers, and the point total was 16. The inspector reviewed 5 children?s records and 3 caregiver record on-site, and 2 household members. This inspection included: document review; tour of the facility; interviews; and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/14/2024. A POC submitted after this date will not appear on the public website.

Household members 14 years of age must obtain the results of a central registry search by the end of the 30th day of becoming 14 years old. The central registry results were received 120 days after HM2 became 14 years old.

Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. The tuberculosis screening for Caregiver, S3, was dated 4 days after the date of hire.
