Inspector Notes

An unannounced, on-site renewal inspection was completed on 10/02/2025. The on-site inspection began at 9:16 a.m. and ended at 12:22 p.m. The inspector reviewed compliance in the areas listed above. There were 5 children present, 2 caregivers, and the point total was 10. The inspector reviewed 5 children?s records and 3 caregiver record on-site, and 3 household members. This inspection included: document review; tour of the facility; interviews; and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/24/2025. A POC submitted after this date will not appear on the public website.

For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service. Caregiver, S3 did not have central registry search results during the inspection and has been employed since June 2023.

Repeat violation
An assistant who is left alone with children in care shall have a current certification in cardiopulmonary resuscitation (CPR) and first aid. Caregiver, S3 observed during the inspection to be providing care of children alone did not have a current CPR and first aid certification.

A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for S3 was due 4 months ago.
