Inspector Notes

A monitoring inspection was initiated on 5/3/2023 and concluded on 5/3/2023. There were a total of 11 children (26 points) observed with 3 caregivers present. The children were engaged in the following: toy play, songs, tracing shapes and numbers, cutting with scissors. The home, 3 child records, 6 caregiver/household member records, evacuation drills, emergency supplies and attendance were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-100-A
Based on a review of children records, the provider has not obtained documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1, first day of attendance 2/21/2023 does not have documentation of a physical examination as of this date, 5/3/2023.
Plan of Correction: The parent was notified and will bring a copy of the physical at pick up today.
Standard 8VAC20-800-180-A
Based on a review of staff records, the provider has not obtained a current report of Tuberculosis screening from each caregiver every 2 years.
Evidence: Staff #1, current TB screening dated 7/27/2020.
Plan of Correction: The staff will schedule a TB screening.
