Inspector Notes

An unannounced monitoring inspection was conducted on April 18, 2024, from 9:05am-10:50 am. At the time of entrance, ( 10 ) children (33 ) points were in care with ( 3) caregivers present. The inspector reviewed 4 children?s records and 2 caregiver/household member records. Children were observed during breakfast, morning circle time and outdoor play time. No medications were on site. Areas of non-compliance are identified on the violation notice.

Based on children's record review, children's records did not contain documentation of name, address, and telephone number of two designated persons to contact in an emergency and no annual review of the children's emergency contact information.
Evidence: The following children records did not have documentation of the following required information;

Child # 2, date of first attendance 9/3/2023, is missing 1 emergency name, address, telephone contact and the annual review of emergency contact information.The last review was dated 1/1/2023.
Child # 3, date of attendance 11/3/2022, is missing 2 emergency name, address, and telephone contacts and the annual review of emergency contact information. The last review was 10/31/2022.
Child #4, date of first attendance 9/30/2023, is missing 1 telephone emergency name, address and telephone contact.
Plan of Correction: Provider will have parents complete the missing information.

Based on children record review, the provider did not obtain documentation of additional immunizations for 1 child.
Evidence: Child # 3, first date of attendance 11/3/2022, age 19 months, does not have updated immunization records. The only immunization record was dated 10/11/2022.
Plan of Correction: The parent will provide the missing immunization record.
