Inspector Notes
An unannounced monitoring inspection was conducted today from approximately 1:00pm-2:00pm. There were 7 children in the direct care and supervision of 2 caregivers. A sample of 3 children?s records, and 2 caregivers records were reviewed. The children were observed eating lunch and preparing for a nap. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Child A was missing documentation of a physical examination.
Evidence:
1) Caregiver A stated the current tuberculosis screening for Caregiver A and Caregiver B were unable to be located.
2) The current tuberculosis screenings for Caregiver A and B expired 12/2021

Caregiver A stated the 2021 training certificates for Caregiver A and Caregiver B were unable to be located.
Evidence:
1) The record of Child A was missing 1 emergency contact.
2) The record of Child B was missing a liability insurance declaration.
