Inspector Notes
An unannounced monitoring inspection was conducted today from approximately 10:45am-11:45am. There were 7 children in the direct care and supervision of 2 caregivers. A sample of 3 children?s records, and 1 caregiver's record were reviewed. The children were observed playing, painting and playing with bubbles. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. The record of Child B was missing documentation of a physical examination.
Evidence:
Caregiver A stated that she could not find a record for herself in the facility.
Evidence:
One unused outlet located near the entrance was not covered with a protective cover.
Evidence:
1) The record of Child A was missing the following required information:
The parents name address and phone number, 2 emergency contacts, medical insurance and policy number, proof of identification, liability insurance waiver, emergency plan.
2) The record of Child B was missing the following required information:
The emergency plan, and field trip permission.
Evidence:
The first aid kit was missing a thermometer.
Evidence:
There were no batteries available for the flashlight at the time of inspection.
Evidence:
Caregiver A stated she could not find documentation of the emergency drills on site.
Evidence:
The record of Child B was missing documentation of immunization.
