Based on record review, the provider did not maintain documentation of the current rabies vaccination. Evidence: The documentation for the rabies vaccination had expired.
Plan of Correction: The vet appointment has been scheduled for this Thursday, 5/16/2024.
Standard 8VAC20-800-80-C
Based on record review the Provider did not document the method of verification of the child's age and identity in 2 out of 3 children's records. Evidence: The records for Child A and Child B did not have the date or name of person viewing the document, to verify the original document had been seen.
Plan of Correction: I will review proof of identity documentation and make sure to write down the date I saw the original document, and sign my name as the viewer.
Standard 8VAC20-800-830-C
Based on record review, and interview with the Provider, documentation for emergency evacuation and shelter-in-place drills did not include the identity of the person conducting the drill, the method used for notification of the drill, the number of caregivers participating, the number of children participating, and special conditions simulated, the time it took to complete the drill, problems encountered, if any, and weather conditions. Evidence: The emergency evacuation documentation only contained the dates of the drills.
Plan of Correction: Moving forward, I will fill out the evacuation logs completely.
