During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 1/17/2025 and completed on 1/17/2025. The on-site inspection began at 11:00am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There was one child present and one caregivers; the point total was 4. The inspector reviewed four children?s records and three caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-120-B
Providers must maintain a record for each caregiver. There was no documentation that two references as to character and reputation as well as competency were checked before employment.for Caregiver 2 who has been caring for children for over six months.
Plan of Correction: The provider will obtain two references for the assistant provider.
Standard 8VAC20-800-60-B
Children's records are required to contain parental employment phone numbers. The record for Child #1 was missing the employment phone number for both parents.
Plan of Correction: Provider will obtain the work phone numbers for the parents.
