During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on 8/21/2025. The on-site inspection began at 9:50am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was eight. The inspector reviewed two children?s records and three caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Household Member #1 was not completed and was overdue.
Plan of Correction: The provider will have the household member obtain an updated TB screening.
Standard 8VAC20-800-260-E
Bathtubs, buckets, and other containers of liquid accessible to children shall be emptied immediately after use. The bathtub in the bathroom that the children use had approximately five inches of standing water, due to a clog.
Plan of Correction: The provider contacted the building manager and was told the clogged pipe would be repaired the next day. The provider will lock the bathroom door until the clog is fixed and will directly supervise the children when they are using the bathroom.
Standard 8VAC20-800-60-B
Children's records are required to contain parental employment phone numbers, and the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record for Child #2 was missing the second emergency contact, and the employment phone number of one parent. The child was present during the inspection and had been in care for over one year.
Plan of Correction: The provider will obtain updated information for the child.
