Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on October 9, 2024, as a part of the licensure period. The on-site inspection began at 9:00am and ended at 10:30am The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 24. The inspector reviewed 9 children?s records and 3 caregiver/household member(s) records onsite. This inspection included document
review, tour of the facility and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in 8V AC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review

The provider has not reviewed with the parent the emergency contact information in the child's record for 15 months.
Plan of Correction: The record was reviewed with the parent.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

For approximately over 60 days child #2 has not have an immunization record on file.
Plan of Correction: The child is exempt from immunizations, I will get the form from the parents.
