Inspector Notes

An unannounced subsidy health and safety inspection was conducted on 4/2/2024 from 12:45 pm- 2:05 pm. At the time of entrance, 4 children were in care with the vendor. The sample size consisted of 4 Children?s records and 2 Provider / Household Member records. Children were observed having applesauce and cereal bars for afternoon snack, participating in a group learning activity, and engaged in free play. Restroom procedures, handwashing and food service was observed. Medication is not administered to children in care and children are not transported in a vehicle while in care.
A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

**** REPEAT VIOLATION ****
Based on a review of records and interview, the vendor did not ensure that caregivers who work directly with children shall, in addition to preservice and required orientation training, annually attend at least 16 hours of training.
Evidence:
One provider record was reviewed.
Provider #1 did not have any documented training for 2023.
The provider confirmed that she did not have documentation of training in 2023 but was able to provide documentation of 1 hours of training in the current year.
Plan of Correction: The vendor has begun taking courses to fulfill training requirements for the current year and will accrue a minimum of 16 hours by the end of December.
Standard 8VAC20-790-420-A
Based on a review of documentation and interview, the vendor was unable to demonstrate that she has a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures.
Evidence:
When asked to produce the written emergency plan for review, the vendor was unable to demonstrate that a written plan had been created.
Plan of Correction: The vendor will complete the Emergency Preparedness Plan template on the department's website to create the written plan by the end of the month.

Based on a review of documentation and interview, the vendor did not practice shelter-in-place drills a minimum of twice a year in 2023.
When asked to review the documented shelter-in-place drills for 2023, the vendor stated that she did not have any documentation of emergency drills conducted in 2023.
Plan of Correction: The vendor will practice shelter-in-place drills a minimum of twice a year in 2024 and document the drills on a log for future review.
Standard 8VAC20-790-430-A-3
Based on a review of documentation and interview, the vendor did not practice Lockdown procedures a minimum of once per year in 2023.
When asked to review the documented Lockdown drills for 2023, the vendor stated that she did not have any documentation of emergency drills conducted in 2023.
Plan of Correction: The vendor will practice at least one lock-down drill in 2024 and document the drill on a log for future review.
