During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on March 23, 2026. The on-site inspection began at 2:35 pm and ended at 3:40 pm. The inspector reviewed compliance in the areas listed above. There were no children present. The inspector reviewed three children?s records and one caregiver record on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the renewal inspection. Subsidy standards that are not covered in voluntary registration standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 6, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-400-H
Medication, except for those prescriptions designated otherwise by a written physician's order, shall be kept locked. Prescription medication was stored on a shelf in the bathroom, out of reach of children.
Plan of Correction: Was out of reach but put it in my pocket that's in locked room.
Standard 8VAC20-850-90-C
Children's records are required to contain parental home information. The record for Child #1 did not have father's home address.
Plan of Correction: Got mom to correct at pick up.
