Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on 5/16/2023. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Standard 8VAC20-790-170-C
Based on record reviews the vendor failed to ensure that the applicant, household member, and any caregiver who is or will be involved in the day-to-day operations of the family day home or is or will be alone with, in control of, or supervising one or more of the children shall (i) undergo a background check in accordance with ? 22.1-289.040 of the Code of Virginia; (ii) shall not have been convicted of a barrier crime as defined in ? 19.2-392.02 of the Code of Virginia; and (iii) is not the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.
Evidence:
1. Vendor #1 and HHM #1 do not have a repeat background check on file. The previous central registry background check was conducted 2/1/2018.
2. Vendor #1 and HHM #1 do not have a repeat sworn disclosure statement on file. The previous sworn statement was dated 2/1/2018.
Plan of Correction: The vendor will ensure that repeat background checks are completed every five years.
Standard 8VAC20-790-200-4
Based on record reviews the vendor failed to ensure that Certifications for first aid, cardiopulmonary resuscitation, and other certifications as required by the responsibilities held by the caregiver.
Evidence:
The vendor and assistant do not have certifications of current first aid and CPR. The first aid and CPR certificate expired 8/22.
Plan of Correction: The vendor is looking for the certificates for First Aid and CPR that was recently completed. Contact is being made with the trainer.
