Inspector Notes

An in-person mandated renewal inspection was conducted on November 22, 2022. There were 5 children (15 points) in care with 1 caregiver supervising. The children were observed playing freely, eating lunch, and sleeping in a crib. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records, 1 staff record and 2 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Based on record review, all household members did not undergo a background check every five years.

Evidence: Household member 1 and 2 have a central registry search which was last conducted on July 13, 2017.
Plan of Correction: I sent out both background checks but do not have the results back yet.
Standard 8VAC20-800-530-F
Based on observation, soft bedding was used under and around an infant while sleeping.

Evidence: Child 1 was observed wrapped up in a thick blanket and sleeping on top of a boppy in a crib.
Plan of Correction: I will not put blankets or pillows in the crib.
Standard 8VAC20-800-60-B
Based on record review, the provider did not sure each child's record was maintained with all required information.

Evidence: Child 1 is missing parent work phone numbers, doctor's name and phone numbers, insurance information, two emergency contacts and written authorization to provide emergency medication attention if needed.
Plan of Correction: I will get the parents information.

Based on observation, caregivers did not wash hands with liquid soup and running water prior to meals.

Evidence:
1) Caregiver 1 did not wash hands prior to giving a child a snack.
2) Caregiver 1 did not wash hands prior to serving and spoon feeling all the children lunch.

Based on observation, caregivers did not ensure children washed their hands with liquid soap and warm running water before eating.

Evidence:
1) Caregiver 1 did not wash a child's hands prior to being offered a cookie.
2) Caregiver 1 did not wash children's hands prior to the children eating their lunch.
Plan of Correction: We will wash hands.

Based on record review, the provider did not provide in writing all required information.

Evidence: Child 1's family was not provided in writing the program's emergency preparedness plan.
Plan of Correction: I will give the parents the information.

Based on observation and record review, children's bottles were not labeled with the name and date.

Evidence: Children's bottles were observed with no name or date.
Plan of Correction: I will label bottles
