Inspector Notes
A mandated monitoring inspection was conducted on May 23, 2023 from 11:30am-12:30pm. There were 7 children (20 pts) with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2child records,1 staff record and 3 household member records were reviewed. Caregivers were observed feeding the children. Children were observed playing freely.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence:
1) Household member 1 has a completed fingerprint based background check on 04/20/2018.
2) Household member 2 has a completed fingerprint based background check on 04/25/2018.
3) Household member 2 has a completed fingerprint based background check on 04/23/2018.
Evidence: Caregiver 1 has a completed fingerprint based background check on 04/23/2018.
Evidence: Household member 1-3 and Caregiver 1 did not obtain a current report of tuberculosis screening from every two years from the date of the first screening. Caregiver stated repeat checks were not completed.
