During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 01/22/2025, as a part of the the provisional licensure period.

The on-site inspection began at 1:25pm and ended at 2:05 pm. The inspector reviewed compliance in the areas listed above.

There were 7 children present and 2 caregivers; the point total was 15. The inspector reviewed 3 children?s records and 2 caregivers records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 02/10/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-280
Potentially poisonous substances shall be stored away from food in areas inaccessible to children.

One air freshener was plugged in near the front entrance on a wall that the children has access to. Three bottles of air freshener spray were located on top of a table next to the dining room.
Plan of Correction: The air freshener has been disconnected from the wall and will not be replaced.
Air freshener sprays will not be accessible to children.
Standard 8VAC20-800-320-B
Child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

Three unused electrical outlets with no protective covers were observed in the dining room area. At the time of entrance, children were taking their nap in the dining room.
Plan of Correction: The dining room will not be accessible to children moving forward.
Standard 8VAC20-800-510-A
A child shall be provided with an individual crib, cot, rest mat, or bed for resting or napping.

At the time of entrance, four children were sleeping on the floor and two children were sleeping on bean bag chairs.
Plan of Correction: Children will be provided with the appropriate sleeping cots.
Standard 8VAC20-800-60-B
The provider shall maintain an up to date record for each child enrolled that contains all the required information.

Child #1's record (enrolled for two months) was missing the name, address and telephone number of two designated persons to contact in case of an emergency; and the Liability insurance declaration form was not signed by the parent. Child #2's (enrolled for 30 months) and #3's (enrolled for 19 months) records were missing the policy number of the child's medical insurance.
Plan of Correction: Child #1?s record has been updated to reflect two emergency contacts.

Child #2 and #3 are updated with medical insurance policy numbers.

Children shall not be allowed to eat or drink while walking, running, or playing.

Children were given candy and they started chewing while walking and playing around in the room.
Plan of Correction: Children will remain seated at the table while eating or drinking.
