Inspector Notes

An unannounced monitoring inspection was conducted on October 31, 2023 between the hours of 9:45 am through 10:45am. There were a total of four children (12 points) observed with two caregivers. A total of six children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, games, hand washing, outdoor play and other organized activities. Areas of non-compliance are identified in the violation notice. Based on record review, required records were not kept for each staff person.
Evidence:
1. Caregiver #2's record (Date of hire-10/16/2023) did not contain documentation of age verification and name, address, and phone number of an emergency contact.
Plan of Correction: The provider will get this information from the staff member.
Standard 8VAC20-800-200-C
Based on record review, documentation of the orientation was not signed and dated by the provider and assistant.
Evidence:
1. Caregiver #2's record (Date of hire-10/16/2023) did not contain documentation of orientation.
Plan of Correction: The staff member will complete documentation of the staff orientation.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #3's record (Start date-09/12/2022) and Child #5's record (Start date-10/24/2022) did not contain documentation of an annual review of emergency contacts information.
Plan of Correction: The parents will review the emergency contact information and document the annual review.

Based on record review, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Evidence:
1. Child #5's record (Start date-10/24/2022) did not contain documentation of additional immunizations. The last immunization was taken on 10/2022.
Plan of Correction: The provider will get a copy of the updated immunization records from the child's parents.
