Inspector Notes

An unannounced monitoring inspection was conducted on Thursday, October 19, 2023. The inspection began at 9:20am and concluded at approximately 12:30pm. There were 133 children present, ranging in ages from 3 months to 5 years, with 20 staff supervising. The children were observed playing on the playground, playing with age-appropriate toys, preparing for snack, and participating in circle time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview with staff, the center did not ensure an annual approval from health department was completed.
Evidence: The most recent annual health inspection was dated 7/11/2022. Staff confirmed that an updated health inspection had not been completed.
Plan of Correction: Health inspection is being scheduled today.
Standard 8VAC20-780-270-A
Repeat Violation - Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence: The gym had torn soft blocks and playmats exposing the inside materials, presenting a choking hazard.
Plan of Correction: Blocks will be repaired or removed from the gym as well as cushion for the couch in the classroom.

Based on record review, the center did not ensure injury reports contained all required information.
Evidence: 2 out of 5 injury reports reviewed, did not contain documentation of how and when the parent was notified.
Plan of Correction: Injury report procedures will be reviewed to assure accuracy with signatures and time.
