Inspector Notes
An unannounced monitoring inspection was conducted on June 10, 2024. There were 175 children present, ranging in ages from 4 months to 10 years, with 26staff supervising. The inspection began at 11:20 AM and concluded at approximately 3:15 PM. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 17 child records and 17 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
4Evidence:
1. The vinyl on a red child size chair and sofa in the Rhino?s classroom was torn on the seat of the chair and sofa exposing the cushion and webbing.
2. Staff stated that they were not aware that the child size sofa and chair was torn.
Evidence: There were containers of diaper rash cream stored in an unlocked cabinet in the elephants classroom accessible to children.
Evidence: In the infant classroom staff was observed conducting a diaper change and the child?s hands were not washed after diaper change. After the diaper change, staff handed the child to another staff member in the classroom.
Evidence:
1. Child #3 (date of enrollment 4/1/2024) did not have the parent?s work number listed in the child?s record.
2. Child #4 (date of enrollment 2/1/2024) did not have the parent?s work number listed in the child?s record.
3. Child #6 (date of enrollment 3/1/2024) did not have the parent?s work number listed in the child?s record.
4. Child #12 (date of enrollment 6/3/2024) did not have the parent?s work number listed int the child?s record.
5. Child #13 (date of enrollment 6/20/2022) did not have the parent?s work number listed in the child?s record.
