Inspector Notes

A monitoring inspection was conducted on June 15, 2022 from 1:15 P.M.-3:45 P.M. There were 31 children present, ranging in ages from two months to five months of age, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that all equipment was maintained in a safe and operable condition.

Evidence:
1. The pink tunnel on the play structure on the playground has a large crack in the middle. The crack spreads off into different directions, and creates sharp edges and pinch points.
2. The playground mat at the end of the slide is buckled in the middle and cracked. The buckling of the mat creates a trip hazard at the end of the chute of the slide.
Plan of Correction: Administration will immediately have the pink tunnel and playground met removed.
Standard 8VAC20-780-330-B
Based on measurement and observation, the center failed to maintain the required depth of resilient surfacing in all the fall zones.

Evidence:
1. At the end of one of the slides and the fall zone around the play structure the mulch was compacted and deteriorated.
2. There was grass and weeds coming up through the mulch in the fall zone areas of the equipment.
Plan of Correction: Administration will immediately have new mulch placed and grass and weeds will be removed.
Standard 8VAC20-780-350-B-1
Based on observations and verification from staff, the center failed to ensure the required staff-to-child ratio for children ages birth-16 months at all times.

Evidence:
1. On June 16, 2022 there were six infants between the ages of 2 months-13 months in care of one staff member. The required ratio is 1:4.
2. Staff verified the number of children in care and the ages of the youngest and oldest child in care.
Plan of Correction: This was corrected immediately. A staff member stayed in the room until naptime was over and the child in question was repositioned back to their designated room. In the future, administration will ensure staff to child ratio will be met at all times.
Standard 8VAC20-780-450-A
Based on observation, the center failed to ensure a top and bottom cover for children 16 months- five years of age, and a bottom fitted cover for an infant.

Evidence:
1. In Classroom #1 there were six children on cots that did not have a bottom cover.
2. In Classroom #2 there were eight children on cots that did not have a bottom cover.
3. In Classroom #3 there were three children on cots that did not have a bottom cover.
4. In Classroom #3 there was one child in a crib that was not provided a fitted bottom cover.
5. Staff #1, and Staff #2 confirmed that the children were not offered a bottom linen for rest time.
Plan of Correction: Administration and staff immediately returned sheets to the children's mats after nap after they were out of the dryer. Staff member were reminded that sheets are to be washed and returned on their beds as soon as they are clean.
