Inspector Notes
Thank you for your assistance during this unannounced monitoring inspection. This onsite visit was conducted on March 29, 2023 from 11:20 a.m. through 1:00 p.m. Upon arrival, there were 23 children ages. 9 months through 4, in the care of 4 staff. Five staff and five children records were reviewed and updated. No medication is being administered at this time.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations
4Evidence:
1. Five staff records were reviewed,
2. Staff #2 did not have documentation of obtaining an updated TB test with two years of the prior one. Last documented TB screening was dated 03/14/2021.
3. The director confirmed Staff #2 had not obtained an updated TB screening.
Evidence:
1. One can of Febreeze air freshener was found in an unlocked cabinet in infant room.
2. Staff confirmed the air freshener was not in a locked cabinet.
Evidence:
1. One tube of Destin was found on a shelf not labeled with the child's name in the infant room.
2. Staff confirmed the Destin was not labeled with the child's name.
Evidence:
1. One can of Similac Formula and one Fruit and Yogurt Smoothie was found on a shelf in the infant room not labeled with the child's name and date.
2. The staff confirmed the items were not labeled with the child' s name and date.
