Inspector Notes

An unannounced monitoring inspection was conducted on September 2, 2023, from 11:30 a.m. through 12:50 p.m. Upon arrival, there were 23 children, ages 6 months through 4, in the care of 4 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 children and four staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation the center failed to maintain a safe condition for children using the playground.

Evidence:

1. Observation of the playground revealed the mat ,at the bottom of the slide, was torn and posed a tripping hazard.
2. Administration confirmed the mat was torn.
Plan of Correction: Administrative Staff removed the mat. Owner will replace immediately. Staff will do daily check to ensure no hazards are on the playground.
