During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on May 19, 2025, as a part of the licensure period. The on-site inspection began at 12:15 p.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 41 children present and 7 staff. The inspector reviewed five children?s and two staff records. This inspection included document review tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 17. A POC submitted after this date will not appear on the public website

Standard 8VAC20-780-160-A
Documentation of a tuberculosis screening shall have been completed within the last 30 calendar days of the date of employment. Staff #1's tuberculosis screening was dated 10 weeks prior to the first date of employment.
Plan of Correction: Staff was sent to have TB test done. She was out ill and brought it back on 5.23.2025.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

One 10 month old infant was observed in a crib with a pacifier in her mouth that was fastened to a beaded holder.
Plan of Correction: The child in the crib was awake. In the future, even if awake, all children will no longer be in the crib with anything. I had all staff sign a safe sleep and shaken baby news letter and went over these safe preventative habits.

Play yards shall not be used as a designated sleeping area. One infant was observed sleeping in a play yard.
Plan of Correction: While deep cleaning to prevent the spread of illness, we closed on the Friday before the inspection. The play yad was used by the employee who brought her children and had them in it to prevent exposure to chemicals. We forgot to fold it up after the weekend. Unfortunately, a child was in it to play and fell asleep. In the future, we will continue to be a play yard free facility as we always have been.
