An unannounced inspection was initiated and completed on January 21, 2026, in response to a complaint received by the licensing office on January 5, relating to allegations of 8VAC20-780 -340, 8VAC20-780-370 and 8VAC20-780-430 This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The informationgathered during the inspection determined no violations with applicable standards or law.No violations were issued.
